To request an evidence of coverage (EOC) during this time, please visit the Authority’s Risk Management Portal and complete the Evidence of Coverage Request Form. If your EOC will need to be issued prior to January 2, please call Insurance Programs Manager Jim Thyden at (562) 631-5449.
